515 Therapy partners with weight loss clinics in Iowa to complete assessments to determine an individual's readiness and eligibility for bariatric surgery. Potential clients can contact our main number, 515-401-1010, to get connected with a therapist who is able to complete the assessment. Assessments can be completed either via Telehealth or in person in our office.
After the appointment, the therapist will complete a letter identifying if the client is a good candidate for surgery. The therapist will fax the assessment to the weight loss clinic. The assessment will be complete within 10 days of the appointment.
