What Is It?

515 Therapy partners with weight loss clinics in Iowa to complete assessments for individuals interested in bariatric surgery. Bariatric assessments include psychological evaluations that help determine a patient's suitability for weight loss surgery. The behavioral health provider evaluates a patient's mental health and emotional well-being. The evaluation may include: clinical interview; questionnaires; discussion of current and past mental health concerns, eating patterns, substance abuse, and trauma; discussion of post-surgical life, including exercise and diet restrictions; discussion of past attempts at weight loss, etc.​

After the appointment, the therapist will complete a letter identifying if the client is a good candidate for surgery. The therapist will fax the assessment to the weight loss clinic. The assessment will be complete within 10 days of the appointment.
